Flu Shot Information for Families!

Diller-Odell Families, flu shot information and permission forms are being sent home! If you would like your child to receive a flu shot at school, please carefully review the information and complete ALL sections of the permission form.

Important Reminder: Completed and signed forms must be returned by the deadline, September 28th. Students without a completed and signed permission form will NOT receive a flu shot.

Please be sure to include a copy of your Insurance or Medicaid card, as required if you have insurance.
