Launch the media gallery 1 playerThis is a reminder that Kindergarten and 7th grade students need to have a physical prior to the first day of 2016/17 school year. Students in Kindergarten must also have a vision exam and provide a birth certificate. While visiting your physician, make sure you have your vaccination schedule up-to-date. Please submit all of the required documentation of physicals and vaccinations prior to school starting in August.
Students with health compromised conditions such as asthma, diabetes, etc. must have a new plan of care for each school year. Asthma and anaphylaxis forms are available in the school office. Ask your physician to sign a plan of care to be on file at school. If you are getting a sports physical, this is a good time to talk to your physician and get their signature on a plan of care. It is a Nebraska State Law that each school has these signed forms on file.
